Each specimen should be placed
in a separate formalin container. The container must be labeled with the patient's
name, MRN, and tissue location/site. Complete the Surgical Pathology requisition
slip which may be used to list up to twelve specimens from the same patient and affix
to the container or place in the pocket on the outside of a biohazard bag.
The following information is required on the Surgical Pathology requisition:
Date specimen
obtained (so we can track the specimen)
Patient's name
(so we can identify the patient)
Medical record
number (double check on patient identification)
Submitting
physician's name and doctor number (so you can get the report)
Diagnostic
code (so HFHS can be reimbursed for the service)
Tissue location/site
(so we can integrate this knowledge in the diagnosis)
Clinical history/pre-post/radiologic dx
Pertinent clinical
information (because diagnostic accuracy is influenced by clinical context;
e.g., HIV immunosuppression)
Additional
copy to physician(s) (to facilitate timely patient care communications)
Special requests;
e.g., stat or date result needed by, special tests, beeper number to notify
of result, etc.
