Collection
of a properly labeled blood sample from the correct patient is crucial
to safe blood transfusion. Blood samples and/or accompanying
request forms which do not meet established criteria for acceptability cannot
be used for testing. Standards for specimen acceptability are enforced
by national accrediting and regulatory agencies, including the College of American Pathologists and the U.S. Food and Drug Administration.
Blood Bank
specimens must be labeled in accordance with the specimen
labeling requirements and the specimen tube label must meet the following
additional requirements:
- Collector's initials or C-number (if
IPD preordered specimen) (Collection Manager ID)
- Valid Sunquest ID number
(if OPD preordered specimen)
- Correct tube submitted
(Pink top EDTA tube)
The
specimen itself must meet the following standards:
- Tube completely filled
- Specimen
less than 72 hours old
- Specimen
not broken, hemolyzed, or otherwise contaminated
- Type and Screen and Confirmatory ABO must be collected at different times.
- CONABO must be collected at a different times (at least 1 minute apart from the prior collection)with a patient identification performed for each draw.

The Blood
Bank "B" Tag (if applicable) must include:
- Complete patient name and
medical record number
- Legible
patient name and medical record number
- Name
and medical record number corresponding to the name and medical record number
on accompanying specimen
- Collector's
signature
- Collector's
name (printed)
