1. For patients who expire at Henry Ford Hospital, fax a copy of completed forms to HFH Autopsy Suite at (313) 916-6471 and then deliver completed forms (including Medical Examiner release) to the Admitting Department (Remains, Release and Decedent Affairs) Tel: (313) 916-7073 prompt 2. Fax: (313) 916-4960.
   
   • For patients who expire at Clinton Township, West Bloomfield or Wyandotte Hospitals:
     Consider Medical Examiner Jurisdiction, contact Pathology Resident ON-CALL at (313) 598-1735, notify HFH Admitting (Remains, Release and Decedent Affairs) at (313) 916-7073 prompt 2, and fax Disposition of Body Form with Autopsy Permit and Consultation Form to two offices:
     
     • HFH Remains, Release and Decedent Affairs at (313) 916-4960 and
     • HFH Autopsy Suite at (313) 916-6471.
     
     Local pathologist on-call is available to provide information and assistance and can be contacted via local hospital operator.
   
   
2. The Admitting Department confirms the permits and contacts the Department of Pathology Central Autopsy at (313) 916-1421 during business hours 7:00 am - 3:30 pm or at the beginning of the next business day.

Note:

• Autopsies are performed as sooon as possible, following transportation arrangements, chart review, triage and legal corrections to the permits if needed. In the case of multiple autopsy requests, the autopsies are performed according to the date and time of death.
• For autopsy issues after hours, on weekends, and/or holidays, contact Pathology Resident ON-CALL at (313) 598-1735 or page the Staff Pathologist ON-CALL via the hospital operator.
